Patient Forms
Patients visiting Thibodaux Surgical Specialists for the first time will be required to complete certain forms pertaining to our office policies, insurance, medical history and how your medical records and other information will be handled (HIPAA). These forms will be provided to you when you arrive at our office. However, you may save time by bringing these completed forms to our office when you come to your appointment.
Your Office Visit
In order to reserve ample time in each individual consultation, we prefer to work by appointment only, except in cases of urgent need. If you need to reschedule your appointment, please give us at least 24 hours notice. This allows us to offer your time to someone who may be waiting.

In case of emergency after hours, call 985.447.5500. Our physicians are on staff at Thibodaux Regional Medical Center, and the operator will contact them for you. If you have a question or medical concern, please don’t hesitate to call. A member of our office staff will be happy to arrange the assistance you need.
Fees & Insurance
We are committed to keeping the costs of our services affordable, while maintaining our high standards of quality care. One way we control administrative costs is by eliminating the necessity of billing for office visits. Therefore, we must request co-payment and deductible applicable for your office visit on the day of services.
Insurance Filing
As a courtesy, we will file your insurance forms with your insurance company for you. We participate in a number of health care plans. Please check with our staff regarding your particular plan.
